Visit Includes:
- Wellness Visit and Breast Exam with mammogram referral if needed.
- Labs: A1C, CBC, CMP, Lipid, TSH, HIV, and PAP with HPV testing if needed.
- Follow-up for abnormal labs.
No Out of Pocket Cost
This is a promotional visit available for the whole month of October. No insurance is required and no copays will be collected. Insurance will be billed if available, but any cost not covered by insurance will be covered by CHHC.
At your appointment you will be entered in a drawing for a grill!
