As an RN Care Coordinator, you’ll provide coordinated care to our patients by developing, monitoring, and evaluating their care alongside our providers and care teams. You’ll report to our Lead Care Coordinator.
What you’ll do
- Meet patients and their families and explain the role of the care coordinator.
- Take part in care meetings, coordinate information and care needs with other care providers, and resolve issues that could slow down a patient’s care.
- Build a complete plan of care that helps each patient reach their milestones, working directly with care teams, patients, and their support system.
- Work with the care team to evaluate outcomes and how well interventions are working.
- Respect patients’ rights and keep their information confidential.
- Help with nursing duties when needed, and other duties as assigned.
What you’ll need
- High school diploma or equivalent. Experienced or certified applicants are preferred.
- CPR certification.
- Two years of experience in a role involving training and care management is preferred.
- Working knowledge of medical equipment and instruments, patient-care policies and procedures, and workplace safety precautions.
- Strong communication, listening, scheduling, and relationship-building skills, and a team-first attitude.
Good to know
- This is an hourly (non-exempt) position.
- The role involves regular sitting, reaching, walking, stooping, and kneeling, and lifting up to 25 pounds.
- Under Oklahoma’s medical marijuana laws, this is a safety-sensitive position involving direct patient care, and medical marijuana use is prohibited.
