As an LPN Care Coordinator, you’ll provide coordinated care to our patients by developing, monitoring, and evaluating their care alongside our providers and care teams. You’ll report to our Lead Care Coordinator.
What you’ll do
- Meet patients and their families, explain the role of the care coordinator, start the care plan, and share educational information with our providers about treatments, procedures, medications, and ongoing care.
- Carry out each patient’s care plan as outlined by their provider, taking part in care meetings and coordinating information with other care providers so care moves forward smoothly.
- Monitor care plans for progress and work with the direct care team to speed up the scheduling and completion of tests, procedures, and consults.
- Work with the care team to evaluate outcomes and how well interventions are working, and put the team’s recommendations into practice.
- Keep the patient care database current by entering new information as it comes in and verifying findings and reports.
- Respect patients’ rights and keep their information confidential.
- Help with nursing duties when needed, and other duties as assigned.
What you’ll need
- A current Oklahoma Licensed Practical Nurse (LPN) license in good standing with the Oklahoma Board of Nursing.
- Graduation from an accredited school of nursing.
- CPR certification.
- One or more years of nursing experience, preferably in a clinic setting.
- Two years of experience in a role involving training and case management is preferred.
- Strong communication, listening, scheduling, and relationship-building skills, and a team-first attitude.
Good to know
- This is an hourly (non-exempt) position.
- The role involves regular sitting, reaching, walking, stooping, and kneeling, and lifting up to 25 pounds.
- Under Oklahoma’s medical marijuana laws, this is a safety-sensitive position involving direct patient care, and medical marijuana use is prohibited.
